Preface

A novel strain of In fl uenza A (H1N1) virus emerged and outbreak of In fl uenza A (H1N1)
occurred worldwide, posing a severe threat to the health of human being. In fl uenza A (H1N1) virus is highly mutable. Research indicated that the pathogenic novel strain appears to be a result of recombined virus gene segments from human in fl uenza viruses and swine in fl uenza viruses in animals before attacking human. Therefore, it has been predicted that the virus gene segments recombination is likely to cause the worldwide pandemic.
In fl uenza A (H1N1) viruses have an average latent period of 7 days. Infected cases, with an acute onset resembling to fl u, show typical symptoms of fever, general upset, muscular pain, sore throat, running noses and cough, sometimes conjunctivitis, nausea, abdominal pain and diarrhea. Cases with consistent high fever should be given special attention. Chest X-ray for critically ill patients shows parenchymal in fl ammation of unilateral or bilateral lungs, some with accompanying pleural effusion. The overwhelming majority of slight cases are reported favorable prognosis while some progress rapidly to death as a result of multiple organs failure such as encephalitis, respiratory distress syndrome, respiratory failure, heart failure, renal failure and infectious shock. Pathological examinations reveal that histological changes are characterized by hemophagocytic syndrome (HPS), including diffuse alveolar damage with accompanying fi brosis, extensive lobular central necrosis of liver, acute renal tubular necrosis, lymphatic failure and increased soluble receptor IL-2 and IL-6 and interferon g. Its diagnosis should be differentiated from in fl uenza, common old, bacterial pneumonia, infectious severe acute respiratory syndrome (SARS), infectious monocytosis, cytomegalovirus infection, chlamydial pneumonia, and mycoplasmal pneumonia.
In the development of In fl uenza A (H1N1) virus, due to histomorphologically pathological
changes, pathology constitutes the basis of diagnostic imaging while diagnostic imaging
fi ndings constitute the demonstrations of pathology. Therefore, based on diagnostic imaging
fi ndings, we attempt to completely and scienti fi cally understand the nature of In fl uenza
A (H1N1) by anatomy, autopsy and pathological examinations. The author collected over 200 cases critical H1N1-infected cases and over 100 cases were selected. For death cases from In fl uenza A (H1N1), autopsy was performed for eight cases after their families’ informed consent. Over 200 reports on pathological and histological analysis as well as immunohistochemical and electroscopic analysis were obtained. This book therefore was compiled by comparing the published data with the present fi rst-hand data. This book addressed the diagnostic imaging of a newly emerging infectious disease based on the six published treaties on diagnostic imaging of HIV/AIDS. The book is intended to provide fi rst-hand clinical data and their scienti fi c analysis, thus serve clinicalpractice for
In fl uenza A (H1N1) prevention and treatment as well as related scienti fi c research.
This book includes a wealth of contents with DR, CT, MR imaging fi gures, pathological
and histological fi gures, immunohistochemical and electroscopic fi gures. Related research,
clinical and education needs can be readily satis fi ed by its detailed data, texts and fi gures, as well as typical cases reports. he data collection was tremendously supported and assisted by hospitals all over the country so that this treaty could be fi nished in such a short period of time. Thanks should be presented to the institutions for their collaboration and cooperation. As the related references about In fl uenza A (H1N1) are scarce both internationally and
domestically, and its pathological anatomy, diagnostic imaging and pathological mechanisms
are still under exploration, our research team could only “wade through the sea by feeling our way” and has made the fi rst domestic attempt in this fi eld. However, mistakes are inevitable. We hope the treaty will elicit more signi fi cant researches and achievements to advance our knowledge about In fl uenza A (H1N1) for its prevention and treatment.
