前言
本書系統闡述了以模擬為基礎的醫學教育的現狀與發展方向，模擬醫學教學的理念、教學設施的設計、建設與運營管理；詳細介紹了模擬醫學課程體系的建設，包括教學內容、教材、師資隊伍的建設等。介紹了新的醫學模擬技術與模擬醫學教學相結合，相關信息化網路教學的套用。模擬醫學教學將成為醫學教育中的重要學科，是集醫學知識、臨床技能以及醫學生的職業素質為一體的綜合教育平台。本書還對全球主要的模擬醫學教學設施以及醫學教學學會進行了介紹，展望了全球模擬醫學教學的未來發展趨勢、套用範圍以及面向公共衛生事業的拓展方向。本書適於全國各高等醫學院校臨床醫學專業教師和學生使用。
模擬醫學教學伴隨著醫學的誕生而開始，中國北宋年間的“天聖銅人”是可考證的歷史上最早的正式的模擬醫學教學設備，為了更好地為患者服務，中外歷代醫者通過各種方式來提高醫生的能力。新時期醫學研究的進步使新科技和新設備大量湧現，醫療服務環境和醫患關係的變化需要醫護人員在接觸患者前進行充分的訓練，而臨床教學資源又因上述原因而短缺，這些原因催生了以模擬教學為基礎的醫學教育的蓬勃興起。以模擬教學為基礎的醫學教育在中國僅僅是起步階段，但發展迅速，政府對教育投入逐年增加，全國各醫學院校對以模擬教學為基礎的臨床技能中心的建立要下很大力量，從硬體設施上完全能夠達到國際先進水平，模擬教學科研也開始起步。作者在美國曾與邁阿密大學的臨床技能訓練中心的主任DrMichael Gordon有過長談，記得他曾說過“Simulator is not stupid,curriculum is stupid”,一個不成功的模擬醫學教學不是硬體設備的錯誤，而是套用方法發生了問題。隨著我國硬體建設逐步提高，如何使用設備、如何開發課程是國內的模擬醫學教學工作者所面臨的主要問題。本書根據當前面臨的社會問題，從醫生在新時期的定位說起，闡述醫生的功能的延伸與醫學教育內容的拓展；重點講述以模擬教學為基礎的醫學教育課程體系、模擬醫學教學設施的建立與套用；從模擬醫學教學第一線工作者的立場來介紹模擬為基礎的臨床能力培養的課程體系與傳統課程設定相融合的方法與體會。天津醫科大學於2009年開始籌建“天津醫科大學國際醫學院模擬醫院”，並於2010年投入使用，在廣泛參考國內外先進模擬教學機構的基礎上，將該設施定名為“模擬醫院”，意在為醫學生樹立一個真實醫院的概念，同時，“模擬醫院”與外科手術學、診斷學教研室分開形成獨立設施。在課程上，“模擬醫院”開放的課程定位於“緊密聯繫臨床課的獨立課程”。建立了完整的模擬醫學教學課程體系，分三個階段，數百學時。本書將詳細介紹該課程體系，經過數年的實踐與改進，已經取得了良好的教學效果。“模擬醫院”及以模擬為基礎的臨床能力培養體系，受到國際上的廣泛關注，被教育部領導評價為“這才是真正的教學改革！”。本書將我們的團隊在過去數年的實踐與經驗匯總成冊，側重於模擬醫學教學的理論、課程設定計畫、教材與師資隊伍建設、模擬醫學教學訓練設施的設計建設以及模擬醫學教學設備配置方面的內容，與國內外同行分享，以求指正，共同發展。2008年愛思唯爾出版社集中了全球模擬醫學教育專家完成了專著Clinical Simulation；2013年，芬蘭與德國也用當地語言出版了兩本模擬醫學教學相關的論著。在全球，與其他學科相比，模擬醫學相關的研究方興未艾，乘國家發展的大好形勢，我國同行關於模擬醫學教學的研究與套用也與世界其他國家站在同一條起跑線上。結合我國及本單位的優勢開發出獨樹一幟且行之有效的課程體系，是非常必要的。在整個醫學教育體系中，只有臨床技能課程能夠從醫學生的入學開設到畢業，而且可以延續到住院醫師培訓、專業醫師及職業生涯培訓。以模擬為基礎的臨床技能教學，不僅是動手能力的培養，也包含了標準化的人文醫學素質的訓練。我們的理想是將模擬為基礎的臨床技能訓練打造成融醫學知識、臨床技能與職業素質相結合的綜合訓練平台，醫學教育改革中的一個新的主流學科將呼之欲出。12本書寫作過程中各位參編老師做出了很大貢獻。承蒙帶我走上醫學教育之路的啟蒙恩師美國托馬斯·傑弗遜大學的醫學院院長DrJoseph Gonnella和筆者多年的摯友美國模擬醫學學會會長DrPaul Phrampus為本書作序，向各位表示深深的謝意。希望同行給予指正、共同交流，促進中國模擬醫學教育事業繁榮進步。
